Coroner

Some deaths must be reported to the Coroner by a doctor, hospital or the police. This is a normal legal process and does not necessarily mean that the death is suspicious. The Coroner is an independent judicial officer whose role is to investigate certain deaths and establish who died, and how, when and where they died.A death may be referred to the Coroner if:

  • The cause of death is unknown.
  • The death was sudden or unexpected.
  • The death was violent or unnatural.
  • The death was linked to an accident or injury.
  • There are concerns relating to medical treatment.
  • The person died while in prison, police custody or another form of state detention.

If the Coroner becomes involved, they may decide that no further investigation is required, request a post-mortem examination or open an inquest. In many cases, the funeral can still take place before the inquest has been held once the necessary paperwork has been issued.

What Happens Next?

If the Coroner decides that no further investigation is necessary, the appropriate documentation will be sent to the Register Office so the death can be registered.If a post-mortem examination is required, the Coroner's Officer will explain the findings and advise you when the necessary paperwork has been completed. If an inquest is opened, the Coroner will explain the process and keep you informed of any hearing dates.


Helpful Information

Although the Coroner's investigation must be completed before certain legal steps can take place, you do not have to wait before contacting a funeral director. Funeral arrangements can often begin while enquiries are ongoing, and your funeral director will liaise with the Coroner's Office where necessary.

Medical Examiner Service

The Medical Examiner Service independently reviews deaths that are not being investigated by the Coroner. A Medical Examiner or Medical Examiner Officer may contact the next of kin to:

  • Explain the recorded cause of death.
  • Answer any questions about the care provided.
  • Confirm that you understand the Medical Certificate of Cause of Death.
  • Advise when the paperwork has been sent to the Register Office.

This service provides an opportunity for families to ask questions before the death is formally registered.


Helpful Information

If the death occurred in hospital, you do not normally need to collect the Medical Certificate of Cause of Death yourself. Once it has been completed and reviewed, it is usually sent electronically to the Register Office. The Bereavement Team will let you know when you're able to arrange the registration appointment.


Tracey's Tip 💙

It can be difficult waiting for a phone call from the hospital, but a short delay is often completely normal while the Medical Examiner reviews the paperwork. If you haven't heard anything after the timescale you've been given, it's perfectly reasonable to contact the Bereavement Team for an update—they'll be happy to explain where things are in the process.

Register Office

The death of a loved one should normally be registered within five days in England and Wales, unless the death has been referred to the Coroner.Before you can register the death, the Medical Certificate of Cause of Death (MCCD) must have been completed by the doctor and reviewed by the Medical Examiner, or the Coroner must have issued the necessary documentation.

What You'll Need

The registrar will ask for information about the person who has died, including:

  • Full name and any previous names.
  • Date and place of birth.
  • Last usual address.
  • Occupation (or former occupation if retired).
  • Marital or civil partnership status.
  • The name and occupation of their spouse or civil partner, where applicable.

If you don't know every detail, don't worry—the registrar will guide you through the process.


At Your Appointment

During the appointment, the registrar will:

  • Legally register the death.
  • Issue the Certificate for Burial or Cremation (Green Form) where appropriate.
  • Explain the Tell Us Once service.
  • Advise how to purchase certified copies of the death certificate.

Additional certified copies can be purchased during the appointment or later if required for banks, pension providers, insurers or probate.

The Tell Us Once service allows several government departments to be informed of the death in one process, including:

  • HM Revenue & Customs (HMRC)
  • Department for Work and Pensions (DWP)
  • DVLA
  • HM Passport Office
  • Local council services

Using this free service can save you time by reducing the number of organisations you need to contact individually.
